Most Mumbaikars get a lipid profile done and breathe a sigh of relief when total cholesterol is under 200 mg/dL. Then they forget about it for another year.
That’s a dangerous habit.

The Standard Lipid Profile: What It Measures
A basic lipid panel measures four things:
- Total Cholesterol — the grand total
- LDL (Low-Density Lipoprotein) — often called “bad” cholesterol
- HDL (High-Density Lipoprotein) — the “good” cholesterol
- Triglycerides — fat circulating in your blood
Normal ranges: Total Cholesterol < 200, LDL < 130, HDL > 40 (men) / > 50 (women), Triglycerides < 150 mg/dL.
This tells you something. But not nearly enough.
What a Standard Panel Misses
LDL Particle Size matters enormously. Small, dense LDL particles penetrate artery walls more easily. Two people can have identical LDL numbers — but one is at 3x higher risk because their particles are smaller and more dangerous.
Non-HDL Cholesterol — total cholesterol minus HDL — is a better predictor of cardiovascular risk than LDL alone. The American Heart Association now considers it a primary marker.
Triglyceride: HDL Ratio is a powerful and underused marker. A ratio above 3.5 in Indians is a strong indicator of insulin resistance and metabolic syndrome. Mumbai’s high-carb, high-stress lifestyle pushes this ratio dangerously high in many urban adults.
Lp(a) — Lipoprotein(a) is a genetically determined cholesterol particle. Elevated Lp(a) significantly increases heart attack and stroke risk — independent of standard cholesterol values. It is not affected by diet or statins. Yet it’s almost never checked in routine tests.
Mumbai’s Unique Risk Profile
Indians have a genetic predisposition to higher triglycerides and lower HDL — even at lower body weights. A Lancet study showed Indians develop coronary artery disease nearly a decade earlier than Western populations. The average age of the first heart attack in urban India is now 53 years. That’s 10 years younger than the global average.
This means Mumbai’s working professionals — the 35-year-old IT manager in Powai, the 42-year-old banker in BKC — cannot afford to just track total cholesterol.
